Garis Panduan Pharmacy Integrated Community Care (PICC)

Pharmacy Integrated Community Care (PICC) is an initiative to optimize the treatment of diabetic patients in the community. PICC includes sharing sessions on medication management, healthy lifestyle practices, and promoting a balanced diet. PICC is implemented through the collaboration of family physicians, medical officers, pharmacists, dietetics officers, nutritional science officers, nurses, assistant medical officers, physiotherapists, and community representatives (e.g.: Know Your Medicine Ambassadors, KOSPEN Volunteer Team).
These Guidelines describe the patient’s selection criteria, terms of reference, as well as the work process and script for each Pharmacy Integrated Community Care (PICC) session as a referral for PICC team members.
