Geriatric Pharmacy Protocol

The Geriatric Pharmacy Protocol is the second edition published under the Pharmaceutical Services Program of the Ministry of Health Malaysia (MOH) in 2024. This protocol serves as the latest reference and guidance for pharmacists in managing  geriatric patients encompassing Medication Therapy Adherence Clinic (MTAC), Ward Pharmacy and Home Medication Review (HMR) in healthcare facilities. It provides a brief introduction to the scope of services and procedures involved, as well as the forms used in the activities conducted.
