Medication Error (ME) Reporting
The Medication Error Reporting System (MERS) is a national-level reporting system introduced by the Pharmaceutical Services Programme, Ministry of Health Malaysia, since 2009.
All healthcare professionals from both public and private sectors are encouraged to report detected medication errors. Reports can be submitted through the following channels:
- Online via the URL: https://sites.google.com/moh.gov.my/mers-bapfkkm/home
OR
- Manually using the Medication Error Reporting Form, which can be submitted to the officer responsible for medication safety practices at the respective facility or mailed to the address stated on the form.
Kindly refer to the Medication Error Reporting System (MERS) 2025 document for procedures on completing and submitting reports online.
