Any suggestion(s) to update medicine information in the MOH Medicines Formulary (MOHMF) can be submitted by completing the MOHMF Information Update Request Form which can be downloaded from this site and uploaded to http://tinyurl.com/cadangfukkm;

All suggestion(s) received will be reviewed by the FUKKM Secretariat and will be presented to the MOHMF Panel for approval (if necessary).
