Consent Form for Treatment Using Unregistered Medicine/Indication (Off Label)

This form must be completed by a consultant/specialist/medical officers who want to use medicine/indication not registered with any regulatory body worldwide. The patient must be explained thoroughly on the medicine and give consent by signing the attached form. Copy of the consent must be kept in the patient's record.

The Qualifying Examination to Practice Pharmacy (QETPP) is pre requisite for registration with the Pharmacy Board of Malaysia (PBM).

The following pharmacy graduates/students from universities recognized by the Pharmacy Board of Malaysia (LFM) are eligible to apply to sit for the QETPP:

University which is provisionally recognized by the Malaysian Qualification Agency (MQA) is allowed to conduct pharmacy degree programme and shall recruit students to enroll into pharmacy degree programme.
