Palliative Pharmacotherapy Guideline for Pharmacists

In line with the Pharmaceutical Services Programme Strategic Plan 2026–2030, an initiative to strengthen Palliative Pharmacy Services has been established with the objective of advancing pharmacists’ knowledge, skills, and competencies in the provision of palliative care.

This guideline serves as a comprehensive reference document outlining the roles and responsibilities of pharmacists in palliative care practice. It encompasses evidence-based pharmacotherapy approaches for symptom management, including the management of palliative emergencies, terminal discharge procedures, and the processes governing the return and disposal of unused palliative medications. The guideline also underscores the importance of robust documentation practices, particularly in view of the utilisation of controlled medications as part of the palliative care pharmacotherapy.

As integral members of the multidisciplinary healthcare team, pharmacists play a pivotal role in optimising palliative care delivery through the provision of clinical recommendations, participation in quality improvement initiatives, and engagement in administrative and governance activities aimed at enhancing the quality, safety, and continuity of care for palliative patients.
