Guideline on Ward Pharmacy Activities, First Edition 2023

The Guideline on Ward Pharmacy Activities First Edition 2023, is the latest guidelines published under the Pharmaceutical Services Program of the Ministry of Health Malaysia (KKM). This guideline includes explanations for every clinical activity carried out in health facilities at the Ministry of Health, including medical history taking, case clerking and case reviewing, clinical pharmacy activity recording, patient referral and medical counseling. This guideline is intended as general guidance and reference for pharmacists on the scope of services, work processes and documentation involved in clinical pharmacy activities in hospitals.
