Protocol Medication Therapy Adherence Clinic-Cardiac Rehabilitation (CR-MTAC) 1ST Edition (Year 2020)

The CR-MTAC protocol aims to  assist pharmacists in initiating and conducting the activities of cardiac rehabilitation in a standardized manner across all Ministry of Health’s (MOH) hospitals/clinics.  It contains workflow of standard operating procedures and documentations that are needed to conduct CR-MTAC.

Anticoagulation MTAC (AC-MTAC) Protocol 2nd Edition

Anticoagulation Medication Therapy Adherence Clinic (AC-MTAC) was formerly known as Warfarin MTAC (WMTAC). Due to the discovery of direct oral anticoagulants (DOACs) such as Dabigatran, Rivaroxaban dan Apixaban in recent years, this has prompted the Clinical Pharmacy Working Committee (Cardiology Subspecialty) to revise and come up with AC-MTAC Protocol.
