| Vancomycin therapeutic drug monitoring (TDM) is common in Penang Hospital. In 2010, a study on TDM testing was done and 46.57% of vancomycin testing was found to be inappropriate. The most common reason was inappropriate sampling time (75.92%) which might lead to inaccurate clinical interpretations and caused treatment failures. It also increased the need to repeat sampling which increased time and cost wastage. The study done to identify the causes and implement remedial measures to improve compliance towards sampling time.

| Previous finding showed that there was high percentage of queried prescriptions encountered at Discharge Pharmacy, TARH. “Queried prescriptions” referring to prescriptions that need further clarification from prescribers following screening. Time taken to handle queried prescriptions will lead to increase in waiting time. It was found that 45.3% of discharge prescriptions were received from Medical wards. The prescriptions contributed to 72.6% from total queried prescriptions encountered.

High cost wastage of expired returned medication may cause financial implications such as high cost of disposing the drugs, as well as the cost of new drugs to be purchased. This problem will also lead to the disturbances to our eco-system.
